Application Checklist

Below is a list of documents that are required when you apply for a mortgage. However, every situation is unique and you may be required to provide additional documentation. So, if you are asked for more information, be cooperative and provide the information requested as soon as possible. It will help speed up the application process.
  • U.S. Citizens: Driver’s License, Passport, other government issued ID
  • U.S. Permanent Residents: Alien Registration Card (“Green Card”)
  • Foreign Nationals: Unexpired I-94, I-797A, H-1B Visa and I-797C
  • Most recent two pay stubs
  • Most recent two years W2 forms. If self-employed 1099 Forms
  • Most recent completed and signed two-year tax returns
  • Most recent two month of bank statements with all pages included
  • Most recent two month mortgage statement
  • Copy of the homeowner’s insurance policy declaration page. Please note: Minimum dwelling coverage dollar amount should be at least your loan amount. Provide the insurance agency’s contact information.
  • If there is an existing home equity line/loan, please provide agreement for the lien/loan
  • The homeowners or condominium association’s contact information, HOA fee statement
  • A copy of ratified contract(signed by both sellers and buyers)
  • Tax Returns (all pages) & Copies of 1040, W-2, 1099 or K-1
  • Limited / General Partnership Returns (Copies of 1065)
  • Sub-Chapter S-Corporation Returns (Copies of 1120S)
  • U.S. Corporation Returns (Copies of 1120)
